You're constantly surrounded by a enormous amount of enemies and you have to stop in one place to fight back, but they can attack you from behind and you probably won't know they're there until it's too late. You can't crouch, run or walk while shooting.
Movie-like camera angles (German/'Dutch' tilt, for example) are indeed a way of creating suspense and unpleasantness, but not the only one. You're confusing atmosphere and gampelay with camera angles.
Tension/stress can be created by making the player anxious, uncomfortable, under pressure, overwhelmed, close to death, easily killable or all of those. It's simply not true that over-ther-shoulder-camera and first-person view can't be used to create tension.
